Request for Incident Records
Obtaining a Copy of a Record
A copy of an incident report may be available by contacting the Public Records Officer by email. You may also complete a request form and send in via mail or fax. Please note that there is a fee associated with a request for incident reports. 

Required Information
Record requests will require the following information:
  • Fire or other non-medical incidents:
    • Date and approximate time of incident
    • Address of incident
    • Type of incident (house fire, water problem, etc.)
  • Medical incident (obtained by the patient in person):
    • Date of incident
    • Address of incident
    • Patient name with picture ID

All other medical incident requests must have the appropriate release documentation available. For more information, please contact the Public Records Officer.
