A copy of an incident report may be available by contacting the Fire Marshal's Office at (253) 946-7248.

Requests will require the following information;

Fire or other non-medical incidents

• Date and approximate time of incident
• Address of incident
• Type of incident (house fire, water problem, etc.)

Medical incident (obtained by the patient in person)

• Date of incident
• Address of incident
• Patient name with picture ID

All other medical incident requests must have the appropriate release documentation available. Contact the Fire Marshal's Office at (253) 946-7248 or prevention.web@southkingfire.org.

THERE IS A FEE ASSOCIATED WITH A REQUEST FOR RECORDS.
